HOW DO I SCHEDULE AN APPOINTMENT?
   To schedule an appointment, please call 570-346-3686, extension 149.

If you are an existing patient and have registered for our Patient Portal, you may request an appointment on-line.

IF I SHOW UP LATE TO MY APPOINTMENT, WILL I  STILL BE SEEN?
   If you show late to your appointment, you may have to reschedule.  

DO YOU OFFER EVENING OR WEEKEND APPOINTMENTS?
   Yes, we offer evening and Saturday appointments upon request.

WILL I ALWAYS SEE A DOCTOR AT MY APPOINTMENT?  
   Depending on schedule availability, you may be seen by one of our Doctors.You may also be scheduled with a Physician Assistant or Nurse Practitioner, who work under the supervision of our Doctors.  
 
WHAT HEALTH INSURANCES DO YOU ACCEPT?  
   We accept all forms of health insurance.

WHAT IF I DON’T HAVE HEALTH INSURANCE?  
   If you do not have health insurance and you are not eligible for Medical Assistance, you will be offered a discounted rate. Payment is due at time of service.
 
DO YOU REQUIRE PATIENTS TO PROVIDE A CO-PAY FOR EACH VISIT?
   Yes, co-pays are due at the time of service.

CAN I PAY MY BILL ON-LINE?   
   No, we do not accept on-line payments at this time.

DO YOU ACCEPT CREDIT CARD/DEBIT CARD PAYMENTS?    
   Yes, we accept debit cards all major credit cards.

WHAT IF I HAVE A COMPLAINT?     
   Complaints may be addressed with our Practice Manager; you may reach her at (570) 346-3686, ext. 129.  

CAN I CHANGE PROVIDERS?  
   Yes. You have the opportunity to see the provider of your choice (schedule permitting). If you would like to change providers, please advise our office staff who will work with you to find a more suitable match.

WHAT IF MY EMPLOYER OR FAMILY MEMBER CALLS THE OFFICE  AND REQUESTS INFORMATION ABOUT MY TREATMENT OR DIAGNOSIS?

    Our staff strictly adheres to HIPAA guidelines. Federal law prohibits us from releasing information to anyone unless you provide written authorization. We are required to release information to the parents and/or legal guardians of children under the age of 14. Please refer to our Privacy Notice for more about how we may use and disclose your protected health information.  

WHAT SHOULD I DO IN AN EMERGENCY SITUATION?  
   If your situation is life-threatening, please call 911 or go to the nearest emergency room.  If your situation is non-urgent and can be handled during normal business hours, please call the office at: (570) 346-3686. A provider is on call 24/7 for non-urgent and non-life-threatening matters.
